Often you are advised Polysomnography/ Sleep Study by your doctor and you have very little clue as to what it is or how is it performed. General inquiry also isn't usually much successful as the test is relatively new and the idea is not much prevalent as of now in the public mind.

In some cases, you try searching here and there and become confused in the various options, different charges and settings given by the few 'contacts' who know about or provide polysomnography or sleep testing services.

​

We understand the same and intend to you guide you regarding the polysomnography process, machines used, prevalent charges and the international recommendations about the same.

•Polysomnography entails overnight recording of:

​

•EEG,

•EOG

•EMG

•Airflow

•Chest/ abdominal movement

•Oxygen Saturation

•Pulse rate

•Cardiac Rhythm

•Body position

•Leg Movements

•Infra Red Video recording.

There are 4 levels of polysomnography (each with different level of sophistication and usefulness):

Level 4 PSG:

​

Uses only two sensors- airflow and oximetry-

 

Comments: practically useless for sleep diagnosis, may be used as a screening device to select patients who need to undergo Level 1 PSG.

​

Level 3 PSG:

​

Uses the following sensors:

​

Airflow

Oximetry

Pulse rate

Thoraco-abdominal movements

Snore sensor

​

Comments: Limited usability as lacks EEG channels, hence can't differentiate between awake and sleep or the different sleep stages. Not able to diagnose mild Obstructive Sleep Apnea cases with accuracy and absolutely no information about the various other sleep disorders commonly comorbid with obstructive apnea. However, if done properly, its a useful and cost effective tool to diagnose moderate to severe OSA patients in a home setting.

Utility: Minimal discomfort; useful in patients with high probability of OSA; Low cost (Budget PSG).

​

Level 2 PSG:

​

Uses the following sensors:

airflow

oximetry

thoraco-abdominal movements

ECG

chin EMG

one or two EOG

one or two EEG

Comments:

​

Most widely prevalent 'sleep study' test done in our country. Has limited benefits. Has the following limitations:

​

  1. full EEG channels encompassing the entire brain is missing, it very often happens that part of the brain is having different activity than the part monitored through the one or two EEG leads placed in this study. Often partial seizures occur at certain areas of the brain and are missed out due to lack of EEG lead placement encompassing the entire brain.

  2. Leg EMG and massateric EMG/ upper limb EMG is missing- hence can't diagnose disorders like bruxism, periodic limb movement, very commonly associated with OSA, which remain undetected and hence often result in treatment failure.

  3. unattended- since no attendant/ technician is present (not hooked to live monitoring of the PSG recording via computer, even when present), a huge amount of data loss occurs, when the various leads get displaced or loose as the patient tosses and turns in bed or makes movement. Also, general comment of the recording technician about various sleep events of the patient is immensely helpful in manual scoring and making the correct diagnosis.

  4. No camera: the lack of an infra red camera and video recording misses out on many nocturnal sleep events like parasomnia, seizure, abnormal movements, pseudo RBD etc.

Utility:

​

Level 2 PSG, however is helpful in the following conditions:

​

old infirm patient, not able to leave home or reach sleep lab for an in-lab level 1 PSG.

In ICU settings.

where in lab Level 1 PSG in not available.

​

Rs. 8000-12000 for 'Home sleep testing' and Rs. 14,000- 18,000 for in Hospital study using Level 2 PSG Machine.

Level 1 PSG: 'Fully attended In- Lab PSG':

​

Uses the following sensors:

Infra Red Video recording of entire night of study

Attended by a trained technician who continuously monitors the tracings and sleep events in computer from the control room.

Comments:

​

Ideal 'Gold Standard' polysomnography capable for diagnosing the entire spectrum of sleep disorders. Coupled with video recording and manual scoring by a board certified Sleep Medicine Specialist or a certified Sleep Technician, its 'THE' Polysomnography needed to diagnose and subsequently treat the entire range of sleep disorders, including even complex, long standing and treatment failure cases.

​

Market Price:

Rs. 13,000-18,000 for In-lab PSG with 'auto scoring' or manual scoring by NON- board certified technician. Also, many centres don't use the camera routinely for PSG studies.

​

  • Level 1 PSG with Infra Red Video with Next day Multiple Sleep Latency Test (MSLT): Rs. 17,990.

  • Level 1 PSG with Infra Red Video with Next day Maintenance of Wakefulness Test (MWT): Rs. 17,990.
